£21,156-£25,557 p/a (£35,260-£42,595 FTE 0.6 per annum) depending on experience

An exciting and rare opportunity for an experienced Occupational Therapist to join our specialist team within St Luke's to help deliver our Strategy “Every Death Matters” with a key focus on early intervention and identification of palliative and end of life care. 



Within St Luke's therapy team their key focus is on rehabilitation palliative care enabling people to live fully until they die. 



The successful applicant will be working predominantly within the specialist community team providing high standard of Occupational Therapy assessment with patients in their own home alongside the Clinical Nurse Specialist team. The AHP team offer an in-reach service to patients admitted to the inpatient Unit, assisting in discharge planning and therapeutic interventions.  



We are seeking an enthusiastic and highly motivated individual to join the multi-disciplinary team in the provision of specialist palliative care to patients and their families.  



The post holder will need to be able to work autonomously and be responsible for complex holistic assessments and planning care to meet patient’s practical, psychological, spiritual, and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues, and good communication and team working skills are essential. 



Whilst the role can be busy and challenging, there is great reward in helping make a real difference to the quality of life and levels of independence to those with life limiting conditions.
